Order our Formaldehyde monitor, catalog #8510 for each room or area that you would like to test.  Each box of personal monitors comes packaged 4 to a box. Monitors are enclosed within a postage paid mailer for convenient return to our laboratory for analysis.

Follow Simple Instructions below:

1. Open the mailer box and remove the white pouch.
2. Open the white pouch and remove the silver pouch.  Tear open the silver pouch and remove the monitor. Note: Please do not destroy or discard any of the packaging, it will be needed for return shipping.
3. On the back of the mailer box, record all of your information. Especially start time and stop time, your contact information for return analysis, and sampling location such as "living room etc."
4. Place the badge near the floor, in the room you would like to monitor with the label facing down. The white and blue "grid" must be facing up to collect ambient vapors.
5. The monitoring time should be as close to 8 hours as possible.  At the end of this time place the badge back in the silver and white pouches and seal both.  Place the pouches back in the mailer and close the box.
6. Make sure that all the information has been recorded, especially the start time and stop time on the back of the mailer box.
7. Place the postage-paid mailer box in the US mail for return shipment.
8. Analysis of the badge will promptly be performed at Kem Medical’s laboratory and a written Laboratory Analysis Report will be sent to you via email.
9. For your own assurance we suggest taking a photograph of the badge in the place during the sampling, "location of testing" and also the completely filled out mailer box which will have the serial number and information you recorded if needed.
